Patient Safety & Employee Opinion Survey

Your opinion matters to us. The Patient Safety and Employee Opinion Survey, which includes 23 patient safety questions and 20 employee opinion questions, takes only 10 minutes to complete. This survey helps us better understand and improve the “safety culture” at UCLA and the environment in which you work. Please go to http://hr.uclahealth.org to complete the survey online. Paper versions may be picked up at various locations. The survey will be available April 6 to May 1. Respondents will be eligible to participate in a raffle.
